About the Role
Elara Caring is seeking a Licensed Practical Nurse Behavioral Health to provide high-quality care in patients' preferred home settings. Responsibilities
- Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines.
- Incorporates patient care goals established in the Plan of Care into nursing visits.
- Communicates significant findings, problems, and changes in condition or environment to the RN Case Manager, physician, clinical supervisor, and/or other personnel involved with patient care.
- Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician.
- Implements the plan for patient safety, using patient, family, and community resources.
- Assists the patient in learning appropriate self-care techniques while delivering quality patient care.
- Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
- Provides services requiring substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
- Collaborates with the RN Case Manager during normal business hours for advice, assistance, and instructions.
- Documents medication regimen and updates medication profile according to established procedures.
- Treats patients and caregivers in the highest and most effective manner.
- Implements all available actions to prevent avoidable hospitalizations and ER visits.
- Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
Requirements
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance and reliable transportation to perform job tasks
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
