About the Role
Lone Star Circle of Care (LSCC) is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) passionate about improving community health. As an LVN, you will support patient care, perform nursing assessments, and assist with examinations and treatments under the direction of medical personnel and Registered Nurses.
Responsibilities
- Obtain, evaluate, and document patient histories
- Measure and record vital signs
- Assist licensed medical staff and/or Registered Nurses during patient examinations
- Draw and collect blood samples and prepare specimens for laboratory analysis
- Prepare and administer medications according to the Five Rights of Medication Administration
- Perform routine diagnostic and laboratory procedures
- Manage referrals, including processing, tracking, and closing referrals
- Receive and document STAT calls and communicate results to providers
- Complete data entry within the Electronic Health Record (EHR) system
- Coordinate authorizations and identify in-network service providers for therapy referrals
- Perform other duties as assigned
Requirements
- Graduate of an accredited vocational nursing program
- Current and valid Texas Licensed Vocational Nurse (LVN) license
- Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
