About the Role

Provide overnight leadership and clinical oversight for pediatric and geriatric clients receiving care in the community. Support clients, families, and field staff during overnight hours to ensure safe, consistent, and high-quality care. This position combines field-based clinical leadership with administrative, quality assurance, compliance, and staff-management responsibilities.

Responsibilities

  • Oversee the overnight care of pediatric and geriatric clients throughout Toronto and the GTA.
  • Visit clients in their homes to assess their care, provide consultations, update care plans, replenish PPE, and conduct supervisory visits.
  • Meet with prospective clients and their families to assess their needs and develop individualized care plans.
  • Introduce caregivers to clients and provide thorough, in-person orientations regarding each client’s routines, preferences, and care requirements.
  • Provide hands-on training, coaching, and mentorship to field staff.
  • Support staff in developing and maintaining competencies for complex medical and personal support needs.
  • Provide real-time clinical guidance and support to overnight field staff.
  • Complete nursing assessments, clinical interventions, and other nursing duties when required.
  • Conduct regular client check-in visits to ensure care needs are met and documentation is accurate.
  • Develop, review, and update individualized client care plans.
  • Complete employee intervention chart audits and follow up on concerns.
  • Conduct employee performance evaluations and provide ongoing feedback, coaching, and support.
  • Complete quality assurance reviews and monitor compliance with policies, standards, and care plans.
  • Prepare reports and clinical updates to support service-frequency extension requests.
  • Step in to cover shifts when necessary to ensure continuity of care.
  • Support evening scheduling team with questions and clinical concerns.

Requirements

  • 4 years minimum of nursing experience
  • 2 years of complex paediatric care
  • Homecare experience (preferred)
  • Previous experience in a leadership role (preferred)
  • Must have paediatric complex care experience with majority of the following interventions: Tracheostomy care, Ventilators, suctioning (oral, nasal and tracheal), G and J tube feeding, Bi-pap or C-pap, Cough assist, Oxygenation, Seizure monitoring
  • Valid Driver's License (mandatory)
  • Strong computer skills, proficient in Microsoft Office
  • Exceptional written and verbal communication skills
  • Intentional organization, planning, time management, and multi-tasking skills
  • Demonstrated experience setting and achieving goals
  • Customer service and leadership
  • Previous teaching or leadership experience an asset
  • Negative TB Test completed within the last year
  • CNO Registration and Registered Nurse (RN) Certificate in good standing
  • COVID Vaccination Receipts subject to an exemption pursuant to the Ontario's Human Rights Code
  • Valid Criminal Record Check - must be completed within the last year.
  • Valid CPR/First aid certification
  • Valid WHMIS certification from the past year
  • Must be able to communicate effectively both verbally and in writing in English
  • Ability to identify and report client care concerns to the team

Salary/Compensations

  • $90,000 to $100,000 (depending on monthly bonus structure)
