About the Role

The Case Manager of Trans Services provides case management services to eligible trans/GNC clients as part of the Trans:Thrive programs and the SF Community Health Center. This role involves conducting client intakes and assessments, developing care plans, and coordinating care with internal and external providers. The Case Manager also offers health education, supportive counseling, crisis intervention, benefits advocacy, and service coordination.

Responsibilities

  • Conduct intake and assessment to eligible clients, provide care plans and supportive meetings to complete goals per individual care plans.
  • Provide case management and supportive counseling for clients engaged in case management programs including, but not limited to Enhanced Care Management (ECM), Trans Well, San Francisco Transgender Alliance for Health Resources (STAHR), House Of Thrive (HOT), etc.
  • Provide information and referrals to services including to primary care, behavioral health, dental, vision, public benefits, managed care plans, shelter and housing, overdose prevention, and employment as well as monitor referral completion.
  • Provide as needed HIV/STI health education and linkage support to PrEP, PEP, DoxyPEP, ART, LAI, and HIV/STI testing.
  • Support with providing as needed resource navigation to clients during Trans:Thrive drop-in.
  • Participate in case conferences, department meetings, monthly staff meetings, in-service training, and agency-wide retreats and outreach events.
  • Complete necessary documentation, assessments, care plans, and encounters in a timely manner, preferably within 24-hours.
  • Participate and comply with agency quality assurance, peer review, and clinical consultation.
  • Perform other duties and tasks as needed.
  • This position requires working occasional weekend and late-night outreach community events on behalf of the agency to engage with targeted populations.

Requirements

  • BA/BS in a health sciences or social work-related field or AAA/CMH/CHW OR comparable work experience.
  • One year of experience providing case management, counseling, advocacy, health education, service coordination, and/or outreach work to disenfranchised communities.
  • At least one year of experience working, or interpersonal experience with transgender individuals and issues.
  • Experience working with diverse populations including trans, BIPOC, the LGBQ communities, refugees/immigrants, people living with HIV/AIDS, people experiencing homelessness, people who are currently or formerly incarcerated, and dual-diagnosed populations (substance use and/or behavioral health issues).
  • Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.
  • Knowledge of HIV, STIs, substance use, mental health, social service and probation systems.
  • Must be adaptive, flexible and a team player.
  • Strong verbal and written proficiency in English.
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills.
  • Familiarity and experience with Microsoft Word and Outlook programs.
  • Detail-oriented with strong follow-through skills, including attention to detail.
  • Must be able to sit, stand, and walk for extended periods of time.
  • Fully vaccinated against COVID-19, complete with booster shot, and able to show proof of vaccination.
  • Up to date with vaccinations, including seasonal flu shot vaccine and TB clearance.
  • Bicultural, bilingual in Spanish (or other SF threshold language)
  • Experience working in a Federally Qualified Health Center (FQHC) or other type of community health centers.
  • Self-starter and self-motivated.
  • PC computer skills preferred, especially with MS Office products.
  • Experience in providing social services or refugee assistance.
  • Knowledge of EPIC, Excel, Zoom and being able to learn other computer programs related to administrative work.
