About the Role

The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is seeking an experienced, dynamic, and highly focused individual to serve within the Education and Workforce Program, supporting participants aged 17-24. This role involves managing a caseload of up to 30 participants, addressing barriers, formulating treatment plans, making referrals, completing documentation, and assisting participants in achieving their goals, with weekly supervision provided by the Clinical Director.

Responsibilities

Complete biopsychosocial assessments for all participants on caseload and identify treatment plans with SMART goals.
Provide weekly/bi-weekly sessions for all participants on caseload.
Maintain notes and treatment plans for all participants in accordance with funder and agency policies.
Identify, predict, and mitigate barriers to self-sufficiency, including economic, education, and employment needs.
Work with participants to achieve educational, workforce, and economic goals.
Determine social service needs, screen for benefits, assist with applications, and make referrals to community agencies.
Provide individualized case management services and support.
Assist individuals with navigating complex systems including healthcare, social services, education, and financial systems.
Facilitate workshops weekly to provide social emotional skills.
Track internal and external referrals of all participants on caseload.
Respond to participants’ needs for mental health, substance use, behavioral, housing, financial, and other social services.
Document and track all interactions with participants through case notes and other tools.
Cultivate strong community-based partnerships with organizations that can serve as referral partners.
Performs other duties as required.

Requirements

At least 1-2 years of human services, education, community development, social work, or relevant experience.
Bachelors degree in human services field.
Strong writing, speaking, interpersonal and organizational skills.
Experience providing case management, facilitating group work, managing data, and effectively coordinating entitlements, benefits, and community resource network, completing treatment plans and monitoring client progress.
Must value team building, be able to work independently, and understand how to actively engage the community to address complex issues.
Must be able to work with a diverse population.
Must be able to effectively multitask and maintain a high level of customer service skill.
Knowledge of EBT as it relates to Trauma Informed Care and interest in expanding their knowledge of other therapeutic interventions.
