About the Role

Provide high-quality case planning, casework, and case management services to at-risk families referred by the Administration for Children’s Services. Focus on intensive preventive services to avoid foster care placement and ensure child safety and welfare, utilizing the Solution Based Casework (SBC) practice model.

Responsibilities

  • Ensure timely completion of SBC training and certification.
  • Conduct bi-monthly home and office visits to assess child safety and risk, complying with ACS requirements.
  • Provide casework and case management services to families receiving Preventive Services (12 cases).
  • Ensure joint-home visits are conducted within 5 business days of referral.
  • Evaluate and assess family functioning, including child and home safety.
  • Partner with families to formulate outcomes and service plans.
  • Complete and process initial intake documents accurately using AAFSC's internal database.
  • Advocate for families by linking them to community services and monitoring progress.
  • Support the facilitation of Family Team Conferences and Service Termination Conferences.
  • Ensure FASPs are launched 30 days prior to the due date and submitted to the Supervisor 3 weeks prior.
  • Complete Initial, Comprehensive, Reassessment FASPs, and Plan Amendments for ADVPO cases offline.
  • Complete and submit Child Trafficking Screening for Child Welfare Services and ADVPO cases.
  • Evaluate walk-ins and community referrals for needed services.
  • Provide case coverage in the absence of a co-worker as needed.
  • Schedule and coordinate monthly visits with FSU workers for FSU cases.
  • Complete all ACS-required documents within 5 business days, including progress notes and FASPs.
  • Complete all ACS and internal forms for case opening/closing and obtain necessary consents.
  • Obtain and maintain collateral contacts with service providers and obtain reports.
  • Comply with data entry requirements for database systems (PROMIS, Connections).
  • Compile and organize client intake paperwork, surveys, and forms.
  • Maintain case files, database entry, and record keeping in compliance with ACS, OCFS, and AAFSC requirements.
  • Follow incident reporting procedures as needed.
  • Maintain relationships with ACS personnel for ongoing supervision.
  • Collaborate and communicate with PPRS staff, responding to emails, maintaining calendars, and participating in meetings.
  • Perform other job-related duties as assigned.
  • Attend required training provided by ACS, Solution Based Casework, and other organizations.
  • Attend internal meetings and collaborate with the multidisciplinary team.
  • Assist in preparation for internal and external audits.

  • Exercises discretion regarding sensitive personal and legal information.
  • Job contingent on pre-employment background checks.
  • U.S. Work authorization required.
