About the Role

Provide case management and crisis intervention services to residents in supportive permanent housing programs with a goal of promoting independent living skills, increased housing stability, and well-being.

Responsibilities

  • Conduct assessments, formulate individualized service plans, and provide referrals and ongoing support to assist tenants in developing skills and achieving personal goals.
  • Connect tenants to community-based services and resources aligned with their Individual Service Plan (ISP).
  • Facilitate support groups and workshops to help tenants live as independently as possible.
  • Serve as a liaison with medical, mental health, and substance abuse providers.
  • Provide assistance with educational and employment linkages.
  • Help tenants overcome barriers affecting timely rent submission.
  • Collaborate with property management to support tenants in maintaining permanent housing.
  • Advocate for and with residents, offering assistance.

Requirements

  • Minimum two (2) years’ experience in delivering clinical crisis services.
  • Computer literate, specifically with Microsoft applications.
