About the Role
This is a fantastic opportunity for an experienced Occupational Therapist to work within a busy Local Authority service, completing person-centred assessments and interventions to promote independence, safety, and wellbeing for adults in the community.
Responsibilities
- Complete comprehensive Occupational Therapy and functional assessments for adults with a range of complex needs
- Assess activities of daily living, mobility, transfers, environmental risks and barriers to independence
- Recommend and prescribe appropriate equipment, minor adaptations and other solutions to support independent living
- Develop practical, person-centred interventions focused on maximising independence and maintaining individuals safely within their own homes
- Work collaboratively with service users, carers, Social Workers, health professionals and wider multidisciplinary teams
- Contribute to care and support planning, risk management and appropriate onward referrals
- Manage a varied caseload effectively, prioritising according to need and risk
- Maintain accurate and timely assessments, reports and professional records
- Apply strengths-based, person-centred practice and relevant health and social care legislation within everyday OT decision-making
