About the Role

Serving persons with mental health conditions, substance abuse issues and/or disabilities, this role is responsible for day-to-day client interactions and care coordination of assigned cases. The goal is to address immediate and emerging needs, set goals, resolve issues, advocate, and connect clients to needed resources for stability and recovery, while enhancing coordination of medical and behavioral health care.

Responsibilities

  • Provide care coordination and educational promotion, including referrals to community linkages and social support services.
  • Maintain compliance with all OMH documentation requirements.
  • Provide advocacy, including assistance seeking and obtaining benefits and entitlements, food, shelter, and permanent housing.
  • Assist recipients in participating in shared decision-making.
  • Facilitate linkage to and systems navigation within behavioral health and allied human services systems.
  • Provide benefits advisement and planning.
  • Assist in the development of psychiatric advance directives (PAD).
  • Assist in advocating for self-directed services.
  • Engage in outreach and engagement activities, including modeling a recovery lifestyle and participating in recovery activities.
  • Raise awareness of existing services and pathways to recovery, and help remove barriers to access.
  • Conduct interim visits with individuals after discharge from hospital emergency rooms, detox units, or inpatient psychiatric units.
  • Assist individuals in selecting and utilizing activities that bring passion, purpose, and meaning into their lives.
  • Coach individuals as they identify barriers to engaging in meaningful activities.
  • Assist individuals in connecting to natural supports that enhance quality of life.
  • Provide recovery education and coaching for individuals and their family members.
  • Offer one-to-one peer support.
  • Facilitate person-centered goal planning incorporating various life areas.
  • Assist with skills development to guide people towards independence.
  • Provide transitional support, bridging individuals from jail, prison, institutions, general hospitals, or their homes to the community.
  • Offer pre-crisis and crisis support, including companionship during emergencies and providing peer support in the home or community.
  • Develop crisis diversion plans or relapse prevention plans.
  • Monitor and document progress of goals and adjust care needs accordingly.
  • Collect and enter data and notes accurately, thoroughly, and timely into the case management system.
  • Attend all assigned trainings and meetings.
  • Perform other duties as assigned.

Requirements

  • Tenacity and passion for this work.
  • Ability to balance objectivity with empathy.
  • Computer literacy required.
  • Valid NYS Driver’s License preferred.
  • A driving record that will permit the use of an Agency vehicle to transport clients.
