About the Role

This is a flexible opportunity for an experienced Occupational Therapist to manage community-based cases through to completion, including assessment, equipment provision, risk management, and major adaptation referrals.

Responsibilities

  • Complete Occupational Therapy assessments and progress cases through to closure.
  • Record assessments and documentation accurately on Mosaic.
  • Obtain manager sign-off prior to case closure.
  • Assess and recommend appropriate equipment where required.
  • Complete risk assessments and reviews to ensure safe and effective interventions.
  • Identify and progress major adaptation referrals when necessary.
  • Work collaboratively with service users, carers, Social Workers, health professionals, and partner agencies.
  • Maintain accurate records and ensure cases are completed within required timescales.

Requirements

  • Occupational Therapy Degree or Diploma.
  • Current HCPC registration.
  • Experience within Adult Social Care, community services, or a Local Authority setting.
  • Strong knowledge of assessment, specialist equipment, and major adaptations.
  • Experience completing risk assessments and reviews.
  • Excellent case management and organisational skills.
  • Ability to work independently and manage cases through to completion.
  • Strong communication, problem-solving, and decision-making skills.
