About the Role

Greater Mental Health of New York’s Care Management Program assists individuals in accessing essential services, including medical specialists, transitioning from hospital admissions, and securing social supports like housing. The Care Manager will deliver comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Rockland County, overseeing medical, behavioral health, substance abuse, and social service needs. This role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices.

Responsibilities

• Manage a caseload of clients consistent with industry standards, regulatory expectations, and client population needs, maintaining a balanced caseload for high-quality care.
• Provide comprehensive care management and care coordination through face-to-face and telephonic services.
• Complete assessments and approve applications for services and entitlements.
• Create individualized plans of care with client input.
• Assist in discharge planning during client hospitalizations and provide follow-up care.
• Consult with team members and other specialists.
• Participate in meetings with community agencies and service providers.
• Follow established agency policies and maintain professional standards.
• Prepare required reports and documentation.
• Provide after-hours emergency coverage and crisis intervention.
• Demonstrate flexibility with working hours.
• Adhere to standards established by regulatory authorities.
• Perform other duties as assigned.

Requirements

• Bachelor's degree in a relevant field (e.g., social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field) or equivalent.
• Two years of relevant experience or equivalent; a master’s degree in a listed education field may substitute for one year of experience.
• Strong computer knowledge including typing, communication tools, scheduling tools, and web browsers.
• Proficiency in Microsoft Office suite and database management systems.
• Fluency in English (Reading, Writing, Speaking).
• Valid Driver's License and satisfactory driving record.
• Adherence to the Agency Driver Policy.
• Maintain New York State Defensive Driving certification within 30 days of hire.
• Maintain pre-employment clearances by applicable regulatory clearances.
• Legal eligibility to work in the United States without sponsorship.

Skills

• Knowledge of behavioral diagnoses and symptomatology.
• Familiarity with community resources.
• Effective client engagement skills.
• Understanding of chronic disease/behavioral health interactions.
• Ability to work independently and efficiently.
• Proficiency in using Electronic Health Record systems.
• Practice in a recovery-oriented, person-centered methodology.
• Fluency in Spanish is preferred but not required.
