About the Role
Work with the CN Guidance FACT Team to deliver services to individuals diagnosed with severe mental illness whose needs have not been met by traditional service delivery approaches.
Responsibilities
- Perform routine team duties and lead responsibility for integrating dual-recovery treatment, family support, or vocational/employment training.
- Provide Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, and incident reporting.
- Coordinate between Forensic ACT and other systems.
- Perform assessments, case management, crisis intervention, and discharge planning.
- Maintain case records and complete documentation on time and in compliance with regulations.
- Provide direct, Telehealth, face-to-face, and/or collateral services in the office or community.
- Work a 35-hour work week, including periodic pager coverage.
- Assist recipients in developing appropriate expectations from treatment.
- Strengthen recipient's ability to understand and utilize therapy and find resources after treatment.
- Exchange information with other agencies for recipient benefit or regulatory compliance.
- Utilize and understand the value of supervision to increase practice skills and promote better services.
- Facilitate sharing of data with individuals to whom it applies for partnered decision-making.
- Complete QI Training during onboarding and participate in refresher training.
- Cultivate a culture of inclusion respecting individual strengths, views, and experiences.
- Adhere to all safety protocols and procedures to ensure a safe working environment.
- Demonstrate commitment to maintaining a safe work environment by following established safety guidelines.
- Apply relevant knowledge or experience regarding health and safety protocols.
- Perform other duties as assigned.

Experience Level
- Two years' experience working with the chronically mentally ill (Masters level) recommended
- Three years' experience working with the chronically mentally ill (Bachelor's level) recommended
- At least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment
Education Level
- Bachelor's degree in social work, psychology, rehabilitation counseling, or related field
- Licensure with the NYS Education Department in a specific field is preferred
