About the Role

This role supports individuals with learning disabilities and autism to achieve their goals, develop independence, and participate fully in their communities. You will empower individuals to build on their abilities, make informed choices, and access opportunities that enhance their quality of life and future aspirations. Working within a strengths-based approach, you will help citizens access community activities, education, employment, volunteering, housing, and independent travel opportunities.

Responsibilities

Develop tailored support plans that promote independence, inclusion, and personal growth.
Help individuals access community activities, education, employment, volunteering, housing, and independent travel opportunities.
Monitor progress through assessment and review processes.
Empower people with learning disabilities and autism to achieve greater independence, choice, and inclusion within their local community.
Develop, implement, and review personalised action plans based on individual strengths, aspirations, and outcomes.
Support citizens to access education, employment, volunteering, housing, life skills, and independent travel opportunities.
Build strong relationships with families, carers, health professionals, housing services, education providers, and community organisations.
Carry out assessments, monitor progress, and coordinate referrals to specialist services where appropriate.
Promote community integration by researching, developing, and maintaining a database of local activities, services, and opportunities.
Work independently in community settings, managing a varied caseload while ensuring care needs are met and risks are appropriately managed.
Apply safeguarding, health and safety, equality, diversity, and data protection principles to protect and support vulnerable adults.
Produce high-quality reports, maintain accurate records, and utilise IT systems effectively, including Mosaic and LifeStar.
Use excellent communication, interpersonal, and problem-solving skills to deliver a high-quality Enablement Service, while embracing Harrow Council's values of Be Courageous, Do It Together and Make It Happen.
