About the Role

Support eligible veterans who self-direct their at-home care to remain living independently in the community. This position assesses veterans’ needs and supports them in developing their self-directed budget and managing their care needs.

Responsibilities

  • Assist participants and their authorized representative in self-directing home and care services within their approved budget.
  • Assist veterans with hiring care attendants, monthly and annual reporting, corporate compliance, and general administrative support.
  • Prepare and maintain data in compliance with policies and procedures.
  • Review veterans’ budgets monthly to ensure they are within budget and address any questions.
  • Ensure the VDC Manager has the appropriate veteran and attendant information for billing.
  • Maintain current copies of the veteran’s case mix budget, Spending Plan, Emergency Backup Plan, and Authorized Representative designation for seven years.
  • Complete and submit quarterly reconciliation reports detailing actual expenses vs. budgeted expenses.
  • Document all interactions with the veteran and authorized representative, including monthly phone calls and quarterly in-home visits.
  • Assist veterans with hiring and paperwork for self-hire staff.
  • Perform ongoing recruitment by developing relationships to market AIM Independent Living Center as a VDC service provider.

Requirements

  • Associate degree in Human Services or Social Work with 4 years of applicable experience, or any combination of education and experience that provides the knowledge, skills, and ability necessary to perform the essential functions of this position.
  • Work is typically performed indoors and requires minimal lifting of up to 10 pounds.
  • Ability to sit and work at a computer for prolonged periods of time.
  • Extensive reading and researching relevant topics.
  • Travel throughout several counties, including Livingston, Yates, Monroe, Wayne, and Ontario.
  • Ability to attend functions in Rochester or other regional- or state-level meetings or functions.
