About the Role

Provides direct and indirect patient care in an office setting, utilizing the nursing process to deliver high-quality, patient-centered care. Determines priorities of care based on patient needs and manages patient flow. Communicates with healthcare providers regarding patient status and diagnostic results, responding quickly to changes in condition.

Responsibilities

  • Support the PCP in the delivery of care by gathering and documenting relevant clinical data.
  • Assist in specialty office procedures as directed.
  • Participate in daily huddles to identify individual patient needs for upcoming appointments.
  • Act as a clinical liaison between patient and provider for questions, concerns, and testing results.
  • Communicate effectively with other members of the care team in coordinating the individualized plan of care.
  • Support PCP during in-office procedures and examinations.
  • Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
  • Perform duties within scope of practice to the highest level of licensure.
  • Provide direct support and guidance to other members of the care team as appropriate.
  • Perform assessment data collection on all patients utilizing tools/forms in EMR.
  • Adequately assess and reassess pain, and educate the patient and family regarding pain management.
  • Provide age-appropriate care to patients in the community setting.
  • Consistently document procedures completed in the EMR, including medications administered and follow-up assessments.
  • Deliver medication utilizing the five rights and observe patients for untoward effects.
  • Maintain current knowledge of medications and their correct administration.
  • Establish priorities of patient care based on needs and office resources.
  • Assist providers with procedures and perform services requiring technical and manual skill.
  • Perform other duties as required.

Requirements

  • Graduate of an accredited school of nursing or nursing program.
  • BLS Certification.
  • Minimum 3 years of nursing experience.
  • Basic computer and EMR knowledge.
  • Excellent communication and interpersonal skills.
  • Ability to assimilate information from many different sources.
  • Must be able to communicate in English both verbally and in writing.
