About the Role

Responsibilities
- Conduct comprehensive assessments and re-assessments of clients, including psychosocial, physical, mental health, environmental, and spiritual needs.
- Develop and initiate client-centered Community Living Plans based on assessment information and program guidelines.
- Conduct home visits, hospital visits, and skilled nursing facility visits, and escort clients to appointments.
- Identify, arrange, and monitor community services using knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain care management relationships with clients and their support networks, providing crisis intervention, advocacy, and problem-solving.
- Meet with clients at least monthly to review and modify their Community Living Plans.
- Document case management activities and identified problems within 24-48 hours.
- Maintain required paperwork and charting systems for continuity of care.
- Evaluate client needs for Purchase of Service and follow up on service provision.
- Educate clients and their support networks about available resources.
- Communicate effectively with community providers, including physicians and social service workers, while maintaining confidentiality.
- Monitor the quantity and quality of services provided by other agencies.
- Evaluate clients' ability to remain safely at home and coordinate placement when necessary.
- Terminate clients appropriately in collaboration with the client, caregiver, and involved services.
- Participate in research studies and data collection as required.
- Participate in Continuous Quality Improvement initiatives.
- Attend and actively participate in team and program meetings.
- Utilize supervision appropriately and maintain open communication.
- Incorporate ethical and legal standards of the National Association of Social Workers.
- Understand and apply the regulatory and procedural requirements of the Institute on Aging.
- Attend continuing education and in-service training to enhance knowledge and skills in case management, gerontology, and related areas.
- Perform other reasonably related responsibilities as assigned.
Requirements
- One year of experience working with disabled adults and/or older adults.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders is highly desired.
- Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems, and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.

Salary/Compensations
- $85,759 to $102,495
