About the Role
The Patient Navigator serves as a patient advocate, guiding patients, families, and caregivers through healthcare system barriers to ensure timely access to quality health and psychosocial care for breast and lung patients. This role facilitates access, builds relationships with referring providers, and manages referrals efficiently.
Responsibilities
- Develop relationships with community groups to create a referral system for patient support, health awareness, and education.
- Track new patient requests and provide monthly statistics and referring patterns.
- Coordinate patient care by removing barriers to healthcare access.
- Resolve patient questions, comments, and concerns, communicating with biopsy patients before and after procedures.
- Communicate with newly diagnosed patients, support scheduling procedures, and connect them with resources and services.
- Obtain and distribute images and reports for second opinions to Radiologists.
- Research new patient resources and treatment options.
- Collaborate with physicians on Breast Patient Navigator and Lung screening program initiatives.
- Interact with patients, other departments, and referring physicians on breast care and lung cancer screening concerns.
- Promote communication between patients and healthcare providers.
- Assist and promote smoking cessation programs.
- Upload, review, and maintain patient data in a tracking system for low-dose screening CT scans.
- Assist in maintaining the lung registry.
- Organize and enter breast and lung pathology into the reporting system.
- Enter patient information into lung history forms.
- Track patients with breast atypia results.
- Maintain a tracker for core-guided breast biopsy patients.
- Obtain reports, scripts, and images from outside facilities for core-guided breast biopsies.
- Contact and assist in scheduling patients overdue for annual screening exams.
- Obtain screening patient scripts and assist uninsured patients with connecting to the New York State Cancer Services Program.
- Complete other tasks and projects as assigned.
Requirements

- Requires computer-related activities for extended periods.

Experience Level
- At least two years of work experience in a healthcare environment dealing directly with patients preferred.
- Technical clinical experience and/or experience in the area of the practice is a plus.
- Experience and competency in a patient advocacy and/or customer service role.
Education Level
- High School Diploma or equivalent required.
- Certification from the National Consortium of Breast Centers required within two years.
