About the Role

The SRO Residential Services Coordinator 3 (RSC 3) provides direct support, advocacy, and education to residents in the Kensington Square program. This role assists residents with developing individualized service plans and coordinates services to enhance their autonomy, quality of life, and recovery process.

Responsibilities

  • Provide tours and lunch visits to prospective residents, offering input on their acceptance into the program.
  • Ensure comprehensive, person-centered service planning for each resident, including recovery-oriented services and discharge planning.
  • Conduct quarterly service plan reviews with residents and their support teams to assess progress and make necessary revisions.
  • Provide teaching, monitoring, and recovery-oriented supports in areas such as Assertiveness/Self-Advocacy Training, Community Integration, Daily Living Skills, Health Services Education, Medication Management, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
  • Observe, monitor, document, and report resident behaviors and progress according to agency standards.
  • Maintain accurate and timely resident files, including progress notes, service plans, and medical records.
  • Schedule annual appointments for physicals, eye exams, specialist appointments, and dental care, arranging transportation as needed.
  • Collaborate with Health Home Care Managers and community service providers to ensure continuity of care.
  • Serve as a liaison between residents and their clinical providers regarding mental health and well-being.
  • Provide direct assistance with daily living skills, including room upkeep, meal preparation, and laundry.
  • Assist residents in planning for the purchase of clothing and other necessities.
  • Encourage healthy connections with family and communities, and help develop new supports and interests.
  • Coordinate and participate in resident recreational activities.
  • Assist Medication Coordinators with medication supervision and assume dining room responsibilities during breakfast when assigned.
  • Provide safe transportation for residents when necessary.
  • Maintain resident confidentiality and demonstrate a positive attitude towards residents, staff, and visitors.
  • Adhere to all safety rules and regulations.
  • Attend required staff meetings and trainings.
  • Contribute as a team member by providing input for treatment/support meetings and continued stay reviews.
  • Perform other duties as necessary for program operation and resident well-being.

Requirements

  • Must meet one of the following education and experience criteria:
  • A. Bachelor's degree in a Human Services field plus at least one year of experience in a Human Services field.
  • B. Associate's degree in a Human Services field plus at least three years of related experience in a Human Services field.
  • C. High School diploma/GED plus at least five years of experience in a Human Services field.
  • Must possess a clean/valid New York State driving record.
  • Must be at least eighteen years of age.
