About the Role

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals diagnosed with severe mental illness whose needs have not been well met by traditional service delivery approaches. The FACT Team provides integrated, evidence-based treatment and support services through an individualized approach, delivered in the community where problems arise.

Responsibilities

  • Assist FACT Team recipients in obtaining and maintaining adequate housing.
  • Understand concrete services such as Medicaid, Medicare, DSS, and SPOA housing.
  • Complete referrals and linkage to community benefits, organizations, and institutions to help clients achieve treatment plan goals.
  • Responsible for Quality Improvement, Utilization Review, and Performance Improvement activities, including data collection, outcome trend analysis, service improvement identification, case record completion, and timely documentation submission.
  • Plan and coordinate all services, in-office or in-field, supporting case management and care coordination, including applications for and maintenance of benefits through the Department of Social Services, Social Security Administration, and housing applications.
  • Facilitate data sharing with individuals to promote partnered decision-making and keep clients informed of progress.
  • Complete QI training during onboarding and participate in QI refresher training as needed.
  • Actively engage in agency-wide trainings, work groups, and NYS-specific trainings on the FACT model.
  • Cultivate a culture of inclusion respecting individual strengths, views, and experiences to drive innovation and better business results.
  • Adhere to all safety protocols and procedures to ensure a safe working environment.
  • Demonstrate commitment to maintaining a safe work environment by following established safety guidelines.
  • Utilize relevant knowledge or experience regarding health and safety protocols.
  • Perform other duties as assigned.

Requirements

  • One year of experience working in a mental health setting at the Masters or higher level, or two years of experience working in a mental health setting at the Bachelors level.
