About the Role

The Population Health Coordinator works directly with patients on population health programs to improve patient care. The coordinator implements clinical data activities, performance improvement, and population health initiatives across various care settings.

Responsibilities

  • Promote a safe, cooperative, and professional healthcare environment.
  • Adhere to all hospital policies, procedures, rules, and regulations.
  • Prioritize customer service, treating all individuals with professionalism, courtesy, and tact.
  • Effectively communicate SBH services and ensure concerns reach the appropriate personnel promptly.
  • Demonstrate standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
  • Implement assigned population health programs, including Prescription Helper, Recertification, Pathfinder, and Living Positive.
  • Collaborate with leadership, management, IT, and stakeholders to collect and process data for clinical care improvement.
  • Produce and distribute chase list reports to guide patient care and enhance quality and performance.
  • Review electronic medical records to identify patients missing services or navigating care transitions.
  • Support quality improvement projects in Population Health and Ambulatory Care services.
  • Contribute to presentations, diagrams, and flowcharts for clinical staff.
  • Perform other duties as assigned by leadership.
  • Attend department and division meetings and participate as appropriate.

Requirements

  • Two (2) years’ experience in healthcare quality settings with data or statistical analysis, data manipulation, and reporting is preferred.
  • Experience working with patients with mental health or substance use issues preferred.
  • Candidates must have good skills with Microsoft Excel.
  • Basic knowledge of vlookup, pivot tables, and formulas required.
  • Excellent written and oral communication skills.
  • Attention to detail and accuracy.
  • Excellent organizational skills.
