About the Role
The mission of the CN Guidance Youth ACT Team is to deliver comprehensive and effective services to individuals and their families diagnosed with severe emotional disturbance (SED) whose needs have not been well met by traditional service delivery approaches. The team provides integrated, evidence-based treatment and support services through an individualized approach, delivered in the community.
Responsibilities
- Assists clients with transportation and representation at court hearings.
- Completes referrals and linkage to community benefits, organizations, and institutions to support treatment plan goals.
- Works with children and families to achieve identified treatment goals through discrete interventions like skill development, training, and education.
- Supports children and families in acquiring skills to manage health, improve relationships, and develop prosocial activities.
- Integrates educational and/or vocational goals and services with team tasks.
- Provides assistance through all phases of vocational services.
- Assists team members with completing assessments such as PHQ-9/a, suicidality assessments, relapse prevention, and safety planning.
- Assists and coordinates planning for community integration trips for Youth ACT Recipients.
- Responsible for Quality Improvement, Utilization Review, and Performance Improvement activities, including data collection, outcome analysis, and service improvement identification.
- Completes case records and ensures timely submission of required documentation.
- Plans and coordinates services that support case management and care coordination for clients and their families.
- Assists with applications for benefits through the Department of Social Services, Social Security Administration, and housing applications.
- Facilitates sharing of data with individuals to support partnered decision-making and inform clients of progress.
- Completes QI Training during onboarding and participates in QI refresher training.
- Cultivates a culture of inclusion respecting individual strengths, views, and experiences.
- Adheres to all safety protocols and procedures to ensure a safe working environment.
- Demonstrates commitment to maintaining a safe work environment by following established safety guidelines.
- Maintains health and safety awareness regarding protocols for a safe work environment.
- Performs other duties as assigned.
Requirements
- One year experience working in a mental health setting at the Masters or higher level.
- Two years' experience working in a mental health setting at the Bachelors level.
Skills
- Understanding of concrete services (Medicaid, Medicare, DSS, SPOA housing).
