About the Role

This role involves improving the health and wellbeing of residents by supporting the development of skills in primary care and adult social care teams to deliver lifestyle advice and Making Every Contact Count (MECC) interventions. The aim is to reduce cardiovascular disease, prevent long-term conditions, and encourage healthier lifestyles. The role also involves leading the development, delivery, and evaluation of training, education, and quality improvement initiatives, providing expert advice to improve preventative health programs, and driving improvements in service delivery to address health inequalities.

Responsibilities

  • Lead the delivery and continuous improvement of the NHS Health Checks Programme across Harrow.
  • Support and develop primary care and adult social care staff to confidently deliver lifestyle advice and Making Every Contact Count (MECC) interventions.
  • Act as a specialist public health nurse, providing expert advice and guidance on disease prevention, health improvement, and best practice.
  • Design, organise and deliver high-quality training, education and coaching sessions for healthcare professionals and partner organisations.
  • Work collaboratively with GP surgeries, adult social care teams and community organisations to improve health outcomes for residents.
  • Use data, evidence and insight to drive service improvements, measure impact and support informed decision-making.
  • Champion preventative healthcare approaches that reduce health inequalities and encourage healthier lifestyles across diverse communities.
  • Lead quality improvement initiatives, supporting providers to meet national standards and achieve positive public health outcomes.
  • Promote culturally competent, compassionate and evidence-based behaviour change approaches that reflect the needs of Harrow’s diverse population.
  • Influence system-wide change by building strong partnerships and strengthening the capacity and capability of the local health and care workforce.

Requirements
