About the Role
As a Care Coordinator - Outpatient Services, you’ll be an important part of helping patients transition from hospitals and facilities to the care they need at home. You’ll work behind the scenes, and sometimes directly with patients - to make sure orders are accurate, insurance requirements are met, and equipment gets where it needs to go.
Responsibilities
- Bring together hospital teams, facility representatives, sales, delivery operations, patients, caregivers, and insurance providers to keep the discharge process moving smoothly.
- Coordinate equipment and supply orders for patients leaving hospitals and facilities, helping create a seamless transition to home.
- Review prescriptions, documentation, and insurance requirements to identify what’s needed to get an order moving forward.
- Create and enter orders into electronic medical record and order management systems while keeping information accurate and up to date.
- Partner closely with hospital liaisons, sales team, delivery operations, billing, and other departments to ensure every patient receives the right equipment at the right time.
- Reach out to patients and caregivers when needed to provide information, answer questions, and help them feel supported throughout the process.
- Ensure orders meet Medicare, Medicaid, and third-party insurance requirements and that all necessary documentation is in place.
- Bring empathy, passion, integrity, and commitment to every interaction with patients, referral sources, and teammates.

- Experience in healthcare, medical billing, customer service, or a related field is strongly preferred.
- Familiarity with Medicare, Medicaid, and third-party insurance is a plus.

Salary/Compensations
- $18.00 - $24.00 per hour
