About the Role

A clinical pharmacist contributes to AHF by expanding primary care functions, providing individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, and nicotine reduction programs. The role enhances AHF's reputation by bringing best practices and collaborative ideas, ensuring consistent application of processes, procedures, and policies. The pharmacist will be a passionate advocate for AHF's top initiatives, keeping core values and mission top of mind in a dynamic environment.

Responsibilities

Provide individualized health promotion and disease prevention.
Administer vaccines, long-acting HIV treatments, and manage diabetes monitoring.
Implement nicotine reduction programs and other subject matter expertise programs.
Collaborate with other AHF Providers.
Bring best practices and collaborative ideas to all locations.
Ensure consistent application of processes, procedures, and policy.
Provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessations programs.
Manage other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority.
