About the Role
The Case Manager will work within our Outreach and Housing Placement Program, providing outreach to homeless people living in or near the North Shore area of Staten Island and responding to community calls regarding outreach to other areas of Staten Island.
Responsibilities
- Connect with homeless people living on Staten Island.
- Make assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc., complete formal assessment whenever possible.
- Assist clients in filling out forms for entitlements.
- Complete psychosocial and social history when applicable.
- Counsel clients leading to interim, transitional, and permanent placements.
- Handle all paperwork and complete accurate timely statistics as required by the program.
- Respond on a timely basis to calls from the community regarding homeless individuals in all areas of Staten Island.
- Drive and escort clients to the ESU and detox as needed using the Outreach van.
- Participate in weekly supervision, case conferences, and staff meetings.
Requirements
- HSD/GED required with 1-2 years experience with the homeless population.
- Experience with the homeless, substance abuse, and mentally ill populations is needed.
- Excellent outreach and client engagement skills are a must.
- Must possess a clean and valid driver's license and the ability to drive a passenger van.
- Must have excellent communication skills both orally and in writing with individuals and groups in regard to complex and sensitive issues.
