About the Role
The Case Aide provides assistance, coordination, and support to the team of Case Planners within the School Based Early Support (SBES) department, under the supervision of the Supervisor. The role focuses on delivering quality support to families and maintaining positive rapport between staff and families.
Responsibilities
- Support Case Planners in service goal planning and outcomes.
- Assist families in connecting with community resources, financial entitlements, mental/physical health services, and educational supports.
- Provide support to families to help them achieve their service goals.
- Maintain contact with families, including conducting monthly in-home or virtual visits.
- Complete documentation in a timely manner, including assessments and case contact notes.
- Maintain an accurate family appointment/field visit calendar.
- Participate in the planning of community events and activities.
- Serve as a liaison for community agencies and school resources.
- Ensure all documentation related to SBES participants and families is kept secure and confidential.
- Assist with developing collaborative partnerships with school staff, administration, and stakeholders.
- Maintain effective daily and monthly communications with stakeholders, school staff, participants, and families regarding events, activities, and SBES programming.
- Perform other related duties as necessary or assigned.
Requirements
- Bachelor’s Degree in Social Work or related field required.
- Minimum of 6 credits in Education, Psychology, or a related field preferred.
- Bilingual preferred.
- Excellent communication skills, both verbal and written.
- Strong computer/technology skill set, including proficiency with Microsoft Office Suite (Word & Excel), CONNECTIONS, and PROMIS ACS.
- Ability to work in a collaborative environment and accept constructive criticism.
- Ability to obtain and maintain all security clearance requirements throughout employment.
- Demonstrated ability to conduct and complete work independently.
- Willingness to take initiative and be flexible.
- Ability to lift 25lbs as requested and needed.
- Ability to observe details at close range.
