About the Role

Provide case management, assessment, treatment, and support services, with a special focus on substance use issues, to individuals with severe and persistent mental illness who are patients of the ACT team.

Responsibilities

  • Provide case management, assessment, treatment and support services, with special attention to substance use issues, to individuals with severe and persistent mental illness who are patients of the ACT team.
  • Conduct clinical documentation in an electronic medical record or client information system.
  • Participate in regular in-service education regarding SHN's comprehensive safety programs and practices.
  • Demonstrate customer service when dealing with patients, colleagues, volunteers, and the community.

Requirements

  • Minimum 3 years recent experience in a community mental health setting working with clients who require careful monitoring and evaluation.
  • Current case management skills with clinical experience of assessing, planning, developing, coordinating and providing treatment, rehabilitation and support services to program clients with severe and persistent mental illnesses.
  • Thorough understanding of the ACTT model.
  • A clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses.
  • Understanding of working with seniors with addictions, cognitive impairments and physical limitations.
  • Strong crisis management skills and proven experience assessing and managing suicide risk.
  • Experience and belief in the value of working in and committing to a fully integrated multidisciplinary team and interdisciplinary model, including clinical guidance and peer support.
  • Good working knowledge of the Mental Health Act.
  • Demonstrated excellence in interpersonal and communication skills that facilitate interaction among all members of the ACTT.
  • Demonstrated superior decision-making and organizational skills.
  • Demonstrated commitment to professional development.
  • Must possess own car, a current Ontario Driver's License and a clean driving record.
  • Demonstrates excellence in communication both verbal and written.
  • Analysis and problem assessment.
  • Demonstrated pattern of good attendance and work record.
  • Demonstrates an understanding of and commitment to SHNs comprehensive safety programs and practices including staff safety, patient safety, and environmental safety.
