About the Role

The Inpatient Social Service Worker is an integral member of an interprofessional team, responsible for providing information, education, and system access and navigation to inpatient clients and their networks of support. This role collaborates closely with the inpatient social worker to address client needs and support continuity of care. The SSW works directly with clients to build essential life skills and promote independence, enhance quality of life, and strengthen their ability to access and engage with community resources. This role emphasizes a strengths-based, client-centred approach that recognizes and builds upon individuals' resilience, capabilities, and existing support.

Responsibilities

Conducts assessments to develop individualized client care plans.
Initiates referrals to community-based services matched to client needs.
Organizes discharge planning meetings and coordinates related activities, ensuring the involvement of the inter-professional team and appropriate community resources.
Collaborates closely with the interdisciplinary team to provide comprehensive support for clients during and after discharge.
Identifies SSW and client directed goals and documents them to plan of care.
Initiates processes related to the client obtaining identification, health card, and any documentation to support their access to health and community-based services.
Documents informed consent provided by the client for the team to engage with their community providers, informal supports, and significant others.
Ensures compliance with Casey House policies, procedures, ethical guidelines, and professional standards.
Provides coaching or educational sessions to help clients develop life skills like budgeting, time management, communication, and decision-making.
Provides harm reduction education and support including safety planning around substance use.
Participates in weekly interdisciplinary team meetings, social work meetings, and care plan rounds.
Networks with community organizations to develop meaningful partnerships.
Maintains professional development through seminars, workshops, and professional affiliations.

Requirements

Diploma in Social Service Work or an undergraduate degree in Social Work.
Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
Experience working with populations living with or at risk of HIV, along with an understanding of the social determinants of health.
A minimum of 3 years of experience working with clients who have complex medical conditions, mental health challenges, and substance use concerns, utilizing a strengths-based approach.
Experience working diverse and historically marginalized populations, including racialized, Indigenous, or 2SLGBTQIA+_communities.
Experience providing case management and advocacy in community-based settings, prioritizing client empowerment.
Excellent communication, advocacy and interpersonal skills.

Skills

Proficiency in using EMR systems and case documentation tools.
Knowledge of EPIC EMR is an asset.
Knowledge of community resources, harm reduction principles, and trauma-informed care.
Strong assessment, service planning, and client advocacy skills.
Demonstrated commitment to harm reduction, health promotion, and social justice.
Experience in facilitating individual and group interventions that build on client strengths.
Ability to work effectively in a fast-paced, interprofessional environment.
Strong communication, interpersonal, and conflict resolution skills centered on client capabilities.
Cultural competency and experience working with marginalized populations, recognizing their strengths and community ties.
