About the Role

Join VillageCare as a Full-Time Nurse Assessor and contribute to the health and wellness of your community by solving complex challenges and providing customer-centric solutions in a supportive environment.

Responsibilities

  • Conduct in-person assessments to evaluate clinical, social, and individual needs for tailored care plans.
  • Collaborate with the assessment scheduling team to confirm sessions and gather information from Members, families, and providers.
  • Review care plans, provide self-care training, and ensure Members have access to resources.
  • Document assessments promptly and work with the Quality Manager and UAS Nurse Reviewer to enhance accuracy.
  • Participate in quality improvement initiatives to elevate service delivery and ensure Member satisfaction.

Requirements

  • Must reside in NY, NJ or CT.
  • Active NYS RN License and valid NYS identification required.
  • At least two years of clinical experience, ideally in home health care or within a health plan.
  • Familiarity with NYS Managed Long Term Care (MLTC) and Medicaid/Medicare plans is beneficial.
  • Proficiency in standard office software applications including MS Word, Excel, Outlook, and PowerPoint.
  • Familiarity with electronic health records systems is a plus.
  • Ability to communicate in languages spoken by VCMAX Members, such as Spanish or Mandarin, is advantageous.
