About the Role

Access Alliance Multicultural Health and Community Services is seeking a diligent, organized, and dedicated individual to join their Primary Care Team. The successful candidate will work within a collaborative, interdisciplinary model of health and wellbeing, providing primary care services within the NP scope of practice at the Danforth Clinic. This role is crucial for addressing population and individual health concerns, ensuring timely, high-quality care for diverse populations including immigrants, refugees, women, 2SLGBTQI+, youth, children, and families.

Responsibilities

Provide primary care to clients of all ages, including assessments, diagnosis, screening, referral, education, treatment, counselling, and follow-up.
Deliver client-centered primary care focused on comprehensive, ongoing health needs, including assessment, management, prevention, health promotion, and continuity of care.
Manage an independent caseload, including triage and determining the need for orders, and interpreting screening and diagnostic tests.
Review client health and collaborate with clients and the team to prioritize treatment, prevention, and health promotion options.
Tailor care approaches to meet the specific health requirements of diverse populations and address health disparities.
Provide appropriate treatment and ensure continuity of care by maintaining complete and accurate client records.
Participate in primary health care program and service delivery within the broader community context, in partnership with other institutions.
Participate in all organizational systems and structures, including quality assurance and performance evaluation.

Requirements

Masters prepared RN (EC) is mandatory.
Active registration and good standing with the College of Nurses of Ontario as a Primary Health Care Nurse Practitioner.
Experience conducting thorough health assessments, including gathering information about relevant medical/family history, pre-existing conditions, and current medications.
Strong cultural competency skills and ability to recognize and respect cultural differences.
Experience with chronic disease management (e.g., diabetes, hypertension) and ability to provide education and support.
Strong interpersonal skills, including active listening, empathy, and effective communication.
Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model.
Ability to work collaboratively within an interdisciplinary health care team.
Strong interpersonal communication skills (written and verbal), problem-solving, organizational, and time management skills.
Strong digital/computer literacy skills and comfort with digital/virtual platforms and service delivery.
Knowledge of issues affecting low income, multi-lingual, and racialized, and 2SLGBTQI+ communities.
Experience with working with low-income, multilingual, multiracial populations and communities.
Ability to speak multiple/second language(s).
Knowledge of immigration policies and laws that affect the population served is preferred.
Experience with trauma-informed care.
Experience in navigation to help clients access resources.
Staff, students, and volunteers must be fully vaccinated against COVID-19.
