About the Role

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program providing coordinated medical and social services to eligible older adults who wish to remain in their homes despite chronic care needs. The Dietary Aide supports food service operations for PACE participants, adhering to dietary plans and organizational policies in collaboration with the PACE Registered Dietician (RD). This role involves preparing, serving, and monitoring meals and snacks, along with other assigned PACE Day Center tasks.

Responsibilities

  • Prepare and serve meals and snacks in accordance with participant dietary plans and PACE program standards.
  • Assist with meal tray preparation, portioning, and distribution to PACE participants during meal service.
  • Ensure participant dietary restrictions, texture modifications, and special diets are followed as directed by the RD or Interdisciplinary Care Team (IDT).
  • Maintain appropriate food storage, labeling, and temperature monitoring practices in accordance with food safety regulations.
  • Maintain a clean, safe, and sanitary kitchen and dining environment in accordance with food safety and infection control standards.
  • Perform dishwashing, kitchen cleaning, and sanitation procedures according to organizational policy and regulatory requirements.
  • Assist the RD with ordering, tracking and delivering meals, snacks, supplies and supplements.
  • Communicate with the Clinical Leader, Speech Therapist and RD regarding nutritional issues or concerns.
  • Understand and adhere to PHH policies and procedures and PHH's Participant Rights Policy.
  • Protect privacy and maintain confidentiality of sensitive Participant and agency information.
  • Attend staff and Participant care planning meetings as requested, and department and PHH in-services as required.
  • Report any variance, accident, or unusual occurrence to the appropriate personnel.
  • Additional work functions and duties may be assigned.

Requirements

  • High School Diploma or equivalent.
  • Must have ServSafe certification or be willing to obtain certification within 90 days of employment.
  • Have 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire.
  • BLS certified or willing to complete necessary certification.
  • Require personal and reliable transportation.
  • Before working independently, meet a standardized set of competencies for the Dietary Aide position established by the PACE plan.
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
  • Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
  • Have not been excluded from participating in Medicare and Medicaid programs.
  • Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards.
  • Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects.
  • Will require 20% sitting, 30% walking, and 50% standing.
  • May also need to move heavy equipment and/or supplies.
