About the Role

The community apartment aide provides support and instruction to tenants in various life skills, ensuring their safety, health, and a clean living environment. This role assists the Service Coordinator in implementing service plans and engaging tenants in on-site and community activities.

Responsibilities

  • Provides support and instruction to tenants in areas such as financial literacy, computer skills, healthy meals, kitchen safety, health and wellness exercises, and other educational programs.
  • Assists the Service Coordinator in developing and implementing person-centered service plans.
  • Completes all tasks necessary to ensure the health and safety of each tenant.
  • Provides enhanced support for tenants identified as high risk.
  • Completes daily documentation and other paperwork promptly and accurately.
  • Attends and participates in staff meetings, in-service training (e.g., incident reporting, crisis intervention), and other agency meetings/trainings.
  • Works collaboratively with the Service Coordinator/Manager and other team members.
  • Records time worked accurately at the start and end of each shift.
  • Safely transports people to and from activities following proper procedures when approved to drive.
  • Ensures adequate supplies are available to support persons served.
  • Complies with all agency policies and procedures.
  • Performs other duties as assigned.

Requirements

  • Must be 18 years of age.
  • High School Diploma or GED preferred.
  • Must possess a NYS Driver’s License that meets agency policy.
  • Ability to read and comprehend plans and documentation.
  • Basic computer skills necessary for communication and documentation.
  • Ability to handle multiple tasks simultaneously.
  • Ability to meet physical requirements including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
  • Ability to lift up to 35 lbs.
