About the Role

As a Social Day Case Manager, you will provide overall support to the Social Day/Day Hub and Congregate Meal Site. Your role involves client intake, ongoing assessment, case management, and coordinating with program staff to ensure high-quality, person-centered services.

Responsibilities

  • Conduct participant intake, evaluations, and ongoing assessments, including required COMPASS assessments and documentation.
  • Manage an assigned caseload of seniors while maintaining accurate client files in accordance with funder requirements.
  • Provide referrals, advocacy, service coordination, and support based on participant needs.
  • Communicate client needs and service plan changes to program staff and provide training as needed.
  • Assist with developing monthly activity calendars, schedules, and programs for clients.
  • Maintain accurate attendance, billing, authorizations, and other required program documentation, including PEER Place and NAPIS records.
  • Assist with daily program operations, including greeting/screening participants, serving meals, purchasing supplies, and supporting activities.
  • Assist with volunteers, students, staff meetings, and required trainings while maintaining effective communication with team members.
  • Demonstrate cultural competence, support The Salvation Army’s mission, and perform other duties as assigned.

Requirements

  • Three to five years of related experience in Human Services (e.g., Social Worker, Case Worker, Program Manager, Coordinator, or similar role).
  • Ability to work as a team member and manage team operations.
  • Ability to provide culturally competent practice.
  • Must have valid NYS Driver’s License that meets The Salvation Army's insurance requirements.
