About the Role

The Flex ACT Wellness Specialist/Behavioral Health Professional Level 1 will monitor and assess individual mental health and wellness status, develop therapeutic relationships, and provide linkage to community services. This role involves ensuring person-centered planning, protecting individual rights, and advocating for individuals receiving services.

Responsibilities

  • Monitor and assess individual Mental Health/wellness status.
  • Identify individual needs based on ongoing assessments.
  • Develop a therapeutic relationship with individuals and assist with problem solving and crisis management.
  • Educate individual and/or family on diagnosis/symptom recognition.
  • Conduct care planning meetings with all relevant service providers.
  • Act as a linkage to community services including medical, behavioral, residential, services, etc.
  • Utilize software technology pertinent to program.
  • Maintain required documentation and deadlines.
  • Ensure that a person-centered planning process is utilized in delivery of services.
  • Link and connect to natural supports and community resources.
  • Ensure that individual rights are protected and advocate for individual.
  • Participates as a team member in the frequent monitoring, assessment and evaluation of the mental health status for individuals; including significant others, the home and the community in which the individuals live.
  • Monitors and coordinates the delivery of necessary services including transportation, making appointments, assistance with housing, law enforcement, social services, employers and doctors.
  • Develops a therapeutic relationship with individuals and assists them with problem solving, crisis management and the development of social support.
  • Provides service coordination/chart management for 10 individuals.
  • Maintains accurate and up to date records for individuals enrolled in ACT.
  • Attends daily team meetings and provides regular updates on status of individuals.
  • Rotates assignment of weekly and weekend after hour’s coverage.
  • Provides technical assistance and education to members of the team in the area of wellness.
  • Develop materials and strategies for wellness self-management and other wellness interventions in conjunction with the team.
  • Adheres to Catholic Charities, NYS Office of Mental Health, ACT Institute and Broome County policies and procedures.
  • Maintains awareness of community resources relevant to individual needs.
  • Attends relevant training and professional meetings and represents agency business in professional manner.

Requirements

  • Ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 25 lbs.
  • Ability to legally operate a vehicle in NY State.

  • Knowledge of mental illness diagnosis and wellness strategies

  • Knowledge of community agencies and resources
  • Ability to work with service providers.
  • Familiar with computer programs and electronic health records.

Experience Level

  • Minimum two years’ experience in behavioral health
  • One year’s experience in substance use setting preferred

Education Level

  • Bachelor’s degree in human services or related field required.

Salary/Compensations

  • $22.05-$24.82/hr.
