About the Role

The Enhanced Care Partner (HHA) provides compassionate, person-centered onsite support to residents living with dementia or physical challenges. This role focuses on promoting dignity, independence, and quality of life through individualized care plans and meaningful engagement.

Responsibilities

  • Provide meal preparation under the direction of the Food Service Manager.
  • Temp and plate food properly using a recipe book.
  • Follow recipes.
  • Clean resident room and common areas, including dusting, vacuuming, floor cleaning, cleaning bathrooms, and keeping the kitchen tidy.
  • Provide recreational activities to residents, according to residents’ preference and under the supervision of the Case Manager.
  • Work collaboratively with household and neighborhood teams to maximize resident potential, comfort, and happiness.
  • Perform residents’ personal laundry.
  • Encourage resident involvement in household life and participation in household tasks.
  • Provide resident care: bathing, oral hygiene, transferring to/from bathroom, use of commode, keeping incontinent residents clean and dry, and changing bed linens.
  • Transfer, ambulate, and position residents to ensure comfort, prevent injury, and promote optimal levels of function.
  • Ensure residents are properly clothed and that clothing is not soiled.
  • Provide personal grooming: trim nails, comb hair, shave male residents daily, and shampoo hair as indicated.
  • Make beds and keep nightstands, dressers, and closets orderly.
  • Assist residents at meals, serve and distribute beverages, assist with cutting foods, provide nourishment, and encourage/monitor meal acceptance.
  • Prepare residents for tests, X-rays, and visits from healthcare professionals.
  • Perform personal services for residents such as writing/reading letters and operating TV/radio.
  • Promote comfort, security, independence, and resident safety.
  • Assist and communicate effectively with relatives, visitors, and guests.
  • Direct inquiries and comments regarding resident health to the nurse in charge.
  • Maintain appropriate techniques in all resident care to prevent the spread of disease and infection.
  • Observe residents and inform the nurse of any changes in attitude, reactions, appetite, or other significant physical changes.
  • Answer call lights and call bells to determine resident needs.
  • Answer telephone calls and relay messages.
  • Follow disaster plan to maintain resident and staff safety during emergencies.
  • Maintain accurate records on resident activities and functions (e.g., TARS, Toileting Schedule, mealtime documentation).
  • Complete documentation in the Electronic system.
  • Maintain a clean, safe, and orderly work area.
  • Report safety hazards.
  • Complete 12 hours of in-service training annually.
  • Practice safe work habits and follow all facility safety procedures.
  • Assure the safety, security, and wellbeing of residents.

Requirements

  • Must be currently certified as a Home Health Aide.
  • Minimum of one year of work experience with the elderly, memory care preferred.
  • Must have High School diploma or two years of related work experience.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant.
  • Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies/staff, and the general public.
  • Must be knowledgeable of practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care.
  • Must possess the ability to plan, organize, develop, implement, and interpret department programs, goals, objectives, policies, and procedures.
  • Must have patience, tact, a cheerful disposition, and enthusiasm.
  • Must be willing to handle challenging residents/associates.
  • Must be able to communicate with staff and other department supervisors.
  • Maintain a rapport with residents and their families.
  • Must be willing to seek out new methods and incorporate them into existing practices.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must attend and participate in continuing educational programs.
  • Must be able to move intermittently throughout the work day.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must meet the general health requirements set forth by the policies of this facility.
  • May be necessary to assist in the evacuation of residents during emergency situations.
  • Must possess sight/hearing senses or use prosthetics that enable these senses to function adequately.
  • Must be able to push, pull, move, and/or lift a minimum of fifty (50) pounds.
  • Must be able to climb stairs.
