Responsibilities
- Engage recipients and create a shared Housing Health and Recovery Plan reflecting person-centered, strength-based recovery principles with the goal of self-sufficiency.
- Provide outreach and assistance in securing apartments within the affordable housing complex.
- Provide services that address all life domains, including employment, finance, relationships, medical needs, health, legal concerns, family, parenting, education, transportation, safety, culture, and spirituality.
- Partner with recipients to integrate into their chosen community and support informed choices regarding available supports, services, and recreational opportunities.
- Assist with discharge planning when recipients are ready to transition from Supported Housing, anticipating and implementing necessary supports and services for a successful transition.
- Serve as a model and guide to other staff on the SHCM team.
- Respond to and intervene in crisis situations as needed.
- Complete required documentation of services provided and assist in orienting new staff.
- Comply with employer regulations and NYS OMH requirements.
- Collaborate with community providers to facilitate smooth transitions for referrals into the Supported Housing Program.
- Work a flexible schedule, including evenings and weekends as needed, to meet recipient needs.
- Provide off-hours on-call and emergency situation response.
Requirements
- 2 years of experience in case management or mental health services, or equivalent experience.
- Excellent interpersonal and engagement skills.
- Valid unrestricted driver's license required.
- Very good organizational skills.
- Must be capable of sitting or standing in front of a computer for long periods.
- Able to work in an open space floor plan.
- Must be capable of moving throughout the workday and following people served throughout the community.
- Work alongside co-workers within 3 feet.
- Must be able to move in tight spaces.
- Occasional lifting of over 25 pounds.
