About the Role
As a Clinician in the Youth ACT program, you will help children avoid hospitalizations and out-of-home placements by providing intensive, community-based services to these youth and families. The youth served by Youth ACT have not found success in traditional mental health and community treatment. Our Clinicians are part of a tight-knit multi-disciplinary team that meets daily to provide medication treatment, therapy, skill building, and psychoeducational support to youth and families to meet all of their needs. You will report to the Director of Youth ACT and you will work on-site and in the community traveling to client's homes required.
Responsibilities
- Provide intensive services to youth and families, meeting clients in the community.
- Participate in specialized training in the Youth ACT model and related best practices.
- Conduct risk assessments, substance abuse screenings/assessments, suicide assessments, and mental health needs assessments.
- Perform social needs screenings and comprehensive psychosocial assessments to develop treatment plans.
- Complete Child and Adolescent Strengths and Needs Assessments.
- Collaborate closely with collateral providers involved in the lives of youth and families.
- Provide holistic care through individual, group, and family counseling.
- Engage in community-based work with flexibility in scheduling.
Requirements
- Master's degree required.
- NYS professional licensure: LMSW, LCSW, LMHC, LCAT or LMFT.
- One year post-graduate experience preferred.
- NYS driver's license required for community-based travel.
