Implements resident programming.
Serves as staff leader during assigned shifts.
Participates in program development, assessment of ILS skills, and program implementation.
Completes documentation for 45 and 90-day reviews.
Manages the ILS portion of the CFA for each individual and annual updates.
Coordinates the annual CFA for each individual.
Manages the Service Plan for each individual.
Purges files to ensure current Program Plans are in place.
Sends information to Day Programs.
Conducts observations at the Day Program site and ICF.
Utilizes Client Track for individual information and Plan of Care.
Functions as Team Leader in absence of the Team Leader.
Trains staff on program areas.
Attends agency-mandated in-service training sessions, including SCIP-R training.
Complies with all ICF/DD and agency policies and procedures.
Supervises residents to ensure their health, safety, and welfare.
Administers medication.
Completes health-related forms.
Attends staff meetings.
Completes all required training in a timely manner.
Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
Performs other duties as assigned.

High School Diploma or GED.
At least one year of experience.
Valid Driver's License that meets agency policy.
Ability to meet physical requirements including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
Ability to lift 35 lbs.
Ability to read and comprehend plans and documentation.
Basic computer skills for communication and documentation.
Ability to lift and transfer individuals manually and mechanically.
Ability to move, reposition, and place individuals in equipment.
Physical agility and ability to react to emergency situations.
Maintain SCIP-R certification.
CPR certification required.
