About the Role

As a Nurse Navigator, you will support members as they navigate different aspects of the healthcare system. You will assess their needs, help coordinate care, connect them with appropriate resources and work with healthcare providers and other stakeholders to help move their care forward.

Responsibilities

  • Assess and support members with their healthcare needs
  • Help members navigate the Canadian healthcare system
  • Coordinate appointments, consultations, diagnostic testing, treatment and follow-up care
  • Communicate with members, healthcare providers, insurers, case managers and other stakeholders
  • Advocate for members and help identify appropriate healthcare resources
  • Identify barriers to care and help develop practical solutions
  • Explain healthcare processes, available resources and next steps to members
  • Maintain accurate and timely documentation
  • Manage multiple cases and priorities while meeting service expectations
  • Work collaboratively with the broader healthcare team
  • Participate in process improvements and service enhancements
  • Perform other duties as required

Requirements

  • Current RN or RPN registration in good standing with the applicable regulatory college
  • Minimum 3 years of nursing experience
  • Strong knowledge of the Canadian healthcare system
  • Excellent verbal and written communication skills
  • Strong organizational and time management skills
  • Excellent critical thinking and problem-solving abilities
  • Strong computer and technology skills
  • Ability to work independently in a remote environment
  • Ability to maintain confidentiality when working from home
  • Compassionate, member-focused approach
  • Ability to manage multiple priorities and deadlines
  • Experience in disability management, case management, care coordination or healthcare navigation
  • Mental health experience or additional mental health training
  • Experience working with insurers, employers, healthcare providers or interdisciplinary teams
  • Experience working with performance measures or service metrics
  • Bilingual in French and English would be considered an asset
  • Dedicated private workspace required
  • Reliable high-speed internet required
  • Ability to maintain confidentiality in a home-based work environment
