About the Role

The Person’s In Need of Supervision (PINS) Caseworker provides intensive home and community-based preventative services for youth ages 11-18 at risk of out-of-home placement, aiming to avoid placement and family disruption. The program helps youth identify underlying issues, learn new skills to reduce risk behaviors, and access community services. The Caseworker will manage a caseload of up to ten youth.

Responsibilities

• Maintain a caseload of referred/enrolled program youth.
• Conduct assessments and intake of referred youth and family within 72 business hours of referral.
• Provide intensive home and community-based services to families.
• Actively participate as part of the treatment team and make referrals to outside agencies in accordance with the treatment plan.
• Obtain regular updates from all service providers to monitor progress and compliance.
• Perform accountability tasks, including compiling and maintaining necessary records.
• Complete all documents required by CHJC and JCDSS in a timely manner.
• Assist in the collection of program data and satisfaction surveys.
• Attend Court hearings/proceedings, monthly meetings with JCDSS, school meetings, and other activities as needed.
• Complete all necessary progress notes, discharge reports, and treatment plans in the Electronic Medical Record (EMR).
• Assess barriers to engagement of youth and family and use problem-solving strategies to overcome them.
• Respond to crises in the home and/or community.
• Provide in-school support as needed and assist youth in getting to school.
• Facilitate/provide Life Skill education to all program youth, individually or in a group setting.
• Seek out volunteer opportunities to promote Community Service and Civic Engagement.

Requirements

• Must maintain privacy of confidential information.
• Successful completion of routine pre-employment requirements mandated by NYS Department of Health, Office of Mental Health, and Office of Children and Family Services, including ongoing clearances and exclusion checks.
• Requirements include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance, and eligibility to work in the United States.
• A valid NYS driver's license is required.
• Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training and Nonviolent Crisis Intervention, First Aid and CPR will be required following date of hire.
• Familiarity with the Family Court System and Department of Social Services.
• Possess written and verbal skills to effectively communicate with co-workers, families, clients, and other service providers.
• Grounded in an understanding of the cognitive, emotional, and chronological maturation of children and adolescents.
