About the Role

Impact Health is seeking a Psych Nurse Practitioner to join our team in Farmingdale, NY. We are a leading advanced practitioner group in skilled nursing facilities, dedicated to prioritizing patient care. We offer a collaborative environment that values expertise and independence, with a flexible Monday through Friday schedule and no nights, weekends, or holidays.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility.
• Provide assessment, diagnostic, and treatment services.
• Provide psychoeducation and medication education to patients and/or families.
• Collaborate with interdisciplinary facility teams.
• Complete timely documentation (within 24 hours).
• Ensure compliance with state regulations in SNF settings.

Requirements

• Licensed PMHNP in the state of practice.
• Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred.
• Minimum of 2 years of clinical experience providing mental health care to adult populations.
• Proficient with EHRs and clinical technology.
• Strong communicator with a collaborative mindset.

Salary/Compensations

• $115,000.00 - $175,000.00 Base+Commission/month
