About the Role
The Housing Coordinator is a peer-based, community-facing role supporting participants in supportive housing programs. Utilizing lived experience and trauma-informed practices, this non-clinical position focuses on helping participants obtain and maintain housing, promote wellness, independence, and community connection. The role involves peer support, housing navigation, tenancy support, documentation, and coordination with various stakeholders.
Responsibilities
- Provide screenings to potential program participants.
- Educate potential program participants on the agency, its mission, and the Supported Housing program.
- Assist accepted program clients with budgeting, including rental costs, utilities, program subsidies, personal finances, and other related costs.
- Explore housing rental options that suit clients' financial, environmental, and personal needs.
- Assist clients in completing rental applications and understanding leasing contract components.
- Assist clients in managing the moving process and furnishing rentals.
- Help clients budget money sensibly to maintain housing and other basic needs.
- Verify client's income and budgeted costs to ensure rental subsidies are up-to-date and accurate.
- Maintain regular meaningful contact with clients, including at least one face-to-face and two indirect contacts per month.
- Educate clients on health & wellness topics, including peer support, recovery from mental health challenges, wellness planning, community resources, and trauma.
- Help clients identify barriers to recovery and wellness.
- Advocate for clients to overcome identified barriers, ensuring their voices are heard.
- Build peer-to-peer connections based on mutuality, empathy, and hope.
- Provide peer support by sharing stories, feelings, information, and strategies for living well.
- Coach clients to identify and accomplish whole health goals.
- Connect people to needed services and supports through direct bridging.
- Develop and maintain positive working relationships with other provider agencies and landlords.
- Document all meaningful interactions in electronic records software and maintain client files.
- Attend SPOA meetings, HUD Continuum of Care meetings, and other related meetings.
- Support the agency's mission to instill hope, empowerment, and self-determination in people living with mental illness.
- Align all behaviors with core values promoting trauma-informed care, engagement, mutuality, and recovery.
- Document time effectively in Kronos and obtain supervisor approval for working over allocated hours.
Requirements
- High School degree or equivalency.
- Personal lived experience with mental illness or substance abuse.
- Significant demonstrable experience in health & human services.
- Must work towards or possess New York State Certified Peer Specialist within 6 months of hire, or Certified Psychiatric Rehabilitation Practitioner (CPRP).
- Must have a clean and valid Driver's License.
