About the Role

Oversee the daily clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, and effective coordination among residents, families, providers, and interdisciplinary team members. This role is for an experienced LPN with strong leadership skills and a passion for resident-centered care.

Responsibilities

  • Oversee the clinical care and daily operations of an assigned nursing unit
  • Supervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidance
  • Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately
  • Coordinate communication with physicians, nurse practitioners, physician assistants, residents, and families
  • Review laboratory results, provider orders, incident reports, and changes in resident condition
  • Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions
  • Assist with admissions, discharges, transfers, and coordination of outside appointments
  • Ensure unit supplies, documentation, and resident records are accurate and current
  • Support staff education, orientation, infection prevention efforts, and quality improvement initiatives
  • Address resident, family, and staff concerns while promoting a positive care environment
  • Perform additional duties as assigned

Requirements

  • High School Diploma or equivalent required
  • Associate's Degree preferred
  • Valid NYS Licensed Practical Nurse (LPN) License required
  • Minimum 1 year of LPN experience in a long-term care setting required
  • 3-5 years of long-term care experience with supervisory experience preferred
  • Strong knowledge of resident care planning, clinical documentation, and long-term care regulations
  • Leadership experience with the ability to supervise, coach, and mentor staff
  • Excellent communication, organizational, and problem-solving skills
  • Ability to coordinate care across multiple disciplines and manage competing priorities
  • Commitment to providing compassionate, resident-centered care

  • +$3.00 shift differential
