About the Role

Sun River Health is seeking a full-time Patient Navigator/Patient Representative to provide patient-centered care, facilitate access to services, and manage the registration and appointment process for patients. This role is crucial as the primary point of contact for patients seeking care.

Responsibilities

  • Assists with daily reception activities and ensures all patient service processes are initiated.
  • Identifies and assists individuals in accessing needed preventive and primary health services, focusing on linking patients to a medical home.
  • Serves as a client advocate, providing assistance and referrals for essential support services.
  • Establishes relationships with community health and human services providers.
  • Communicates effectively with team members and patients regarding all aspects of service.
  • Manages telephone and mail contact with patients.
  • Schedules appointments, collects fees, and explains services and charges.
  • Distributes patient satisfaction cards, patient rights information, and other relevant materials.
  • Verifies insurance information and sets fees based on the sliding scale.
  • Facilitates and monitors scheduling and arrangements for medical and dental appointments.
  • Provides follow-up services for newly diagnosed individuals requiring additional care.
  • Offers basic health information and education.
  • Conducts follow-up to confirm service receipt, track referrals, and help patients overcome barriers to care.

Requirements

  • High school diploma/GED required.
  • 2-4 years of relevant work experience.
  • Bilingual proficiency (oral and written) preferred.
