About the Role
Provide intensive services to families whose children are at risk of removal or are already in placement. The program aims to prevent out-of-home placement, facilitate safe returns, and prevent future placements. You will report to the Program Coordinator.
Responsibilities
- Provide home and community-based assessment, planning, and treatment with individuals, groups, and families enrolled in Family Preservation services.
- Complete all necessary records and client information following agency, county, and state policy and funding requirements.
- Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family.
- Responsible for clinical decisions when paired with a Bachelors Clinical Case Manager.
- Ensure provisions of regular treatment plan meetings and services plan reviews.
Requirements
- Minimum of one year of post-graduate experience.
- Must have a valid driver's license.
