About the Role

Horizon is seeking a Clinical Care Specialist to provide care coordination, work with clinical teams, conduct toxicology tests, respond to crises, and review patient records. The role involves supporting patients and the clinical treatment team, participating in data collection, and ensuring follow-up with behavioral health and medical providers.

Responsibilities

  • Provide various forms of care coordination by relaying medication statuses with patient/counselor and following up on completion of releases, paperwork, and status update reports, and/or applications.
  • Work closely with multidisciplinary clinical teams.
  • Conduct toxicology tests with patients.
  • Respond to calls and walk-ins presenting at the clinic in crisis situations.
  • Review and flag patient records to ensure required updates are prepped and/or made timely.
  • Successfully complete internal training and credentialing process to perform approved clinical services such as educational and process group activities.
  • Relate to children/adolescents presenting with mental health and/or Substance use concerns, and their families of diverse ethnic, cultural, and socioeconomic backgrounds, interacting in a positive, supportive, and cooperative manner.
  • Complete records requests (disability requests/attorney requests/patient request, etc).
  • Monitor clinical and non-clinical data via internal Electronic Health Record (EHR), internal dashboards and reports, as well as regional informational health systems.
  • Provide support and triage to patients relative to the importance of following up with behavioral health and medical providers, as appropriate.
  • Offer support to patients being seen for medication only services.
  • Support the clinical treatment team including providers and clinicians, including follow-up and outreach for engagement purposes.
  • Participate in data collection with active patients.

Requirements

  • Experience working in a computerized office environment with word processing, database and spreadsheet skills sufficient to prepare correspondence, reports, and forms, including the willingness to learn new software applications.
  • Ability to relate to children/adolescents presenting with mental health and/or Substance use concerns, and their families of diverse ethnic, cultural, and socioeconomic backgrounds.
  • Mobility required with repetitive wrist, hand, and finger movements.
  • Handling light duties, occasionally lifting objects up to 20 pounds.
