About the Role
The Rehabilitation Specialist will help clients cope with the effects of their disabilities as they relate to independent living. This role focuses on providing rehab assessments and developing client strength-based psychosocial rehabilitation recovery approaches.
Responsibilities
- Providing hands-on support with living skills and education in groups and individually.
- Participating in multidisciplinary team meetings.
- Completing necessary documentation including assessments, recovery plans, progress notes, and discharge summaries.
- Developing curriculums and facilitating group experiences for clients on multiple topics.
- Providing case management and housing-related services to residential clients to facilitate their adjustment and integration into the residence and community.
Requirements
- At least 4 years of experience in a therapeutic setting providing support services to SMI individuals.
- Ability to perform in a busy, changing, multi-tasking work environment.
- Valid driver's license in good standing.
