About the Role
Assist in the coordination of the residential program for individuals we support, emphasizing independence, integration, individualization, and productivity with a focus on personal choice. This role requires a progressive and proactive approach to providing, arranging, supervising, and documenting services to promote increased independence. The position also involves supporting the agency's goals, objectives, philosophy, and values.
Responsibilities
- Assist individuals we support in purchasing household equipment and managing personal funds.
- Help individuals we support with nutrition, meal planning, preparation, and grocery shopping.
- Provide guidance and instruction in residence cleaning, domestic tasks, and room upkeep.
- Ensure the physical plant is maintained in a safe and sanitary manner.
- Offer guidance on leisure activities and assist with transportation arrangements.
- Conduct and document fire drills as required.
- Communicate person-supported and household supply needs to the Residence Manager.
- Participate in staff meetings regarding resident and household needs.
- Actively participate in crisis intervention.
- Implement and follow through on ADL, interpersonal skill training, and behavioral support programs.
- Document program implementation and resident progress accurately and on the same day.
- Administer medications if designated, after completing the required course and maintaining certification.
- Attend and achieve 80% competency on required in-service training exams.
- Report any illness, accident, or changes in a person's medical status to the on-call nurse or administrator.
- Complete Nursing Notes daily to reflect monitoring of resident health status.
- Write incident reports (GERs) as defined by Incident Review Procedures.
- Ensure the safety and welfare of individuals we support, promoting integration, individualization, independence, and productivity.
- Communicate matters concerning the person's support program and physical plant to the Residence Manager or Assistant Residence Manager via daily log or contact.
- Accept temporary or permanent transfers within the department as needed.
- Adhere to policies and procedures outlined in the Functional Plan.
- Document service plan documentation per regulations and training, including programming, nursing, clinical services, and self-care.
- Monitor individuals we support during sleeping hours and attend to their needs.
- Perform daily cleaning duties as required by the cleaning schedule.
- Complete all cleaning logs and physical plant checklists.
- Act as designated in charge in the absence of the Residence Manager, if designated.
- Follow correct procedures for the use of personal funds, ensuring proper purchasing and documentation.
Requirements
- High School Diploma or equivalency preferred.
- Experience working with persons with a developmental disability preferred.
- Valid/acceptable NYS driver’s license.
- Authorized to work in the United States.
- Must be progressive and proactive.
- Must support the goals, objectives, and philosophy of the agency.
- Must support the Agency’s values by working with measurable signs of PRIDE.
- Staff shall be cleared by the transportation department to drive an agency vehicle and maintain their driving certification.
- If hired as non-driver or certification is lost, there will be a reduction in pay.
- Must successfully complete the OPWDD approved medication course if required to administer medications.
- Must maintain medication administration certification if required.
- Must demonstrate proficiency by written examination on all required in-services by achieving 80% competency on the exams.
- Must use the appropriate documentation procedure and follow instructions given for reporting medical status changes.
- Must write incident reports (GER’s) as defined by Incident Review Procedures.
- Must communicate to the Residence Manager or Assistance Residence Manager all matters concerning the person’s support program and physical plant.
- Must accept temporary or permanent transfers within the department as necessary.
- Must remain aware of and follow policies and procedures as outlined in the Functional Plan pertinent to the assigned work site.
- All CR/IRA staff will be responsible for documenting service plan documentation per regulations and training.
- Must perform cleaning duties on a daily basis as required on the cleaning schedule.
- Must complete all cleaning logs and physical plant checklists as designated.
- Must be aware of and follow correct procedures for the use of personal funds, ensuring proper purchasing and documentation.
