About the Role

The Occupational Therapist evaluates and treats residents in accordance with physician orders, utilizing therapeutic procedures and assistive devices to maximize functional potentials. The role involves participating as a member of the interdisciplinary team, providing support and information within the scope of Occupational Therapy.

Responsibilities

  • Review resident information and assess its impact on assessment and treatment.
  • Conduct and document thorough functional assessments using standardized procedures, including home/vocational and equipment needs.
  • Develop and implement individualized treatment plans with achievable functional goals.
  • Comply with evaluation, treatment, and documentation guidelines, maintaining accurate records.
  • Enter progress notes after each treatment session.
  • Revise treatment plans as needed.
  • Document treatment course in a clear, concise discharge summary.
  • Instruct assistants in treatment procedures and provide supervision.
  • Orient residents to program services and treatment procedures, informing them of potential risks.
  • Collaborate with nursing staff, residents, and families to maximize resident's functional abilities.
  • Instruct residents and families in exercises and therapeutic procedures for post-discharge continuation.
  • Assist in the referral process for residents requiring post-discharge services and maintain knowledge of community resources.
  • Attend and participate in rehabilitation team conferences regarding resident progress, problems, or needs.
  • Attend other department/facility meetings as required.
  • Access continuing education opportunities to maintain clinical currency.
  • Participate in quality improvement processes by responding to audits, surveys, safety activities, and regulatory requirements.
  • Act in compliance with Heritage Healthcare’s regulatory and professional standards.
  • Provide input on the clinical performance of assistants to the Rehabilitation Manager.
  • Promote Heritage Healthcare programs and services through interactions.
  • Adhere to company and facility policies and procedures, participating in quality improvement and safety programs.
  • Perform other duties as assigned to provide quality care.
  • Understand reimbursement by various payers and allocate therapy resources efficiently.
  • Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination Policy, and third-party reimbursement guidelines.
  • Support the company’s commitment to Compassionate Care and project a professional image.
  • Perform duties independently and recognize situations requiring supervision, offering assistance willingly.
  • Maintain expected productivity levels.

Requirements

  • Bachelor/Masters of Science degree in Occupational Therapy.
  • Current license/certification in the state of practice.
  • Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population.
  • Certification, if applicable, and demonstrated competency in identified areas of specialization.
  • Excellent oral and written communication and interpersonal skills.
  • Demonstrated ability to assess resident needs and develop and implement a comprehensive plan of care.
  • Knowledge of accreditation standards and compliance requirements.
  • Ability to safely complete activities including standing for prolonged periods, pushing up to 35 lbs, lifting up to 50 lbs independently (or over 50 lbs with assistance), stooping, reaching, and balancing.
