About the Role
Case Manager working for the Orange County Improving Families Program. The successful applicant will possess stellar credentials in working successfully with youth, families and communities to safely manage troubled families in their homes with intensive case management and wrap-around support.
Responsibilities
- Initiate, organize, plan, develop, and ensure implementation of direct services to assigned participants and families.
- Develop all service plans based on a strength-based approach using the wrap-around model.
- Aid youth and families in crisis using child welfare and neighborhood-based approaches.
Requirements
- Bachelor's degree required.
- Experience working with at-risk youth and families.
- Proficient in Microsoft Office Suite.
- Experience using an electronic health record system is a plus.
- Strong communication and interpersonal skills.
- Position requires reliable transportation, valid driver's license, and auto insurance coverage.
- Bilingual (Spanish Speaking) is a plus.
